The project shall consist of one lump sum base bid proposal along with alternate bid proposal for the following work:

Provide LVT flooring and rubber base in nine (9) classrooms.

All proposals shall be enclosed and sealed in an envelope and have a space on the outside of the envelope properly filled in with the name of the Contractor, name of the trade, name of the job and date of opening.

All contractors submitting bids must submit a completed Contractor’s Qualification Statement

(AIA Document A305) and a minimum of three (3) public school district references where the contractor has completed work of a similar nature in the past five (5) years. This information must be submitted with the Bid Proposal and shall be considered a required Bid Document. Failure to do so may subject bidder to disqualification.

All proposals shall be delivered to the Office of the Superintendent, Burnham S D #154.5, Burnham Elementary School, 13945 South Greenbay Avenue, Burnham, Illinois on or before 2:00 P.M. Prevailing Time, on the date of the bid opening. No proposals will be accepted after that time and date. Bid opening shall be on May 18, 2021, and all proposals shall be opened at 2:00 P.M., Prevailing Time, at the Office of the Superintendent.

Contractors submitting bids shall be notified after Bid Opening as to the Bidder’s List and Base Bid Proposal by all Contractors.

Contract documents may be obtained by submitting a deposit of Fifty Dollars ($50.00) per set, made payable to the Sarti Architectural Group, Inc., 40 Adloff Lane, Suite 6, Springfield, Illinois 62703. Plan deposit is refundable if the contract documents are returned with a post mark dated within thirty (30) days from issuance of the Notice of Award. Upon request, a PDF copy of the Documents will be provided with no deposit required.

A maximum of three sets shall be issued to Contractors and one complete set to Subcontractors and Suppliers. Additional sets may be obtained at a non-refundable cost of $50.00 each. Sets will be mailed to applicants for an additional flat handling-and-mailing charge of Ten Dollars ($10.00) per set, for which a separate check shall be issued, payable to the Architect.

No Pre-Bid Meeting will be held. Contractors who wish to walk through the work areas may contact Mr. Steve Geraci, Superintendent, 708-862-8636, to schedule a time at which the walk through may be performed.
