NOTICE TO CONTRACTORS CLASSIFIED LEGAL ADVERTISEMENT COMMONWEALTH OF MASSACHUSETTS EXECUTIVE OFFICE FOR HEALTH AND HUMAN SERVICES DEPARTMENT OF MENTAL HEALTH – FACILITIES MANAGEMENT General Bids Submission Deadline: 2:00 P.M. Wednesday January 12, 2022 The Category of Work is: ELEVATOR Project Name: 2022-041 Elevator Maintenance, Service, and Repairs Project Location: Allen Hall 288 Lyman St. Westboro, MA 01581 Estimated Construction Cost: $72,000 The Elevator Contractor shall maintain and repair an elevator and two lifts at Allen Hall for a period of approximately 29 months (February 1, 2022 to June 30, 2024). Minimum rates of wages to be paid on the project have been determined by the Commissioner of the Division of Occupational Safety under the provisions of Sections 26 and 27, Chapter 149 of the General Laws and will be included in the bid package. Proposals will ONLY be submitted online via COMMBUYS website by the General Bids Submission Deadline date on forms furnished by the Department of Mental Health (DMH) and clearly identified as a bid, endorsed with the name and address of the bidder, and the project name. Each general bid proposal must be secured by an uploaded photocopy of an accompanying deposit of 5% of the total bid amount, including all alternates, in the form of a bid bond, a certified, treasurer’s, or cashier’s check issued by a responsible bank or trust company made payable to the Commonwealth of Massachusetts. Awarded vendor will then mail the original 5% deposit bond or check to the Department of Mental Health Engineering and Facilities Management Department at 167 Lyman Street Room 158 Westborough, MA 01581. The Department reserves the right to waive any informality in or reject any or all Bids if it is in the public interest to do so. Plans and Specifications will ONLY be available at COMMBUYS from 12/22/21 until bid opening as amended. Bid must be submitted electronically on COMMBUYS by the General Bids Submission Deadline date above.
