REQUEST FOR PROPOSAL

Title: CCTV and Sonar Inspection of Pipes Penetrating Levees

Project Description: Inspection of interior gravity pipes and pump station discharge pipes that penetrate under or through flood protection systems. This work shall include cleaning the pipes, gate wells and headwalls. Pipes and gate wells shall be jetted and vacuumed and the headwalls shall have all debris removed to the end of the headwall. Some headwalls may require temporary sandbagging to ensure that the work be completed properly. All inspections shall follow the attached “Guidance for CCTV and Sonar Inspection Pipes Penetrating Levees” issued by the US Army Corps of Engineers dated February 2013. Submittal Requirements: Contractor shall submit a cost based upon the detail and specifications provided. Contractor shall provide the unit cost for the following items:

1. Highland Segment /s/ $_____ LS

2. Munster Segment /s/ $_____LS

3. Hammond Segment /s/ $_____LS

4. INDOT Segment /s/ $_____LS

Contractor will be required to provide a Certificate of Insurance prior to beginning work. The work shall be completed by the March 1, 2022 once the weather allows.

Contractor is encouraged to participate in the MWBE Commission goals as adopted in Resolution Number 2015-04.
