PUBLIC NOTICE SEALED PROPOSALS will be received by the Hazle Township Board of Supervisors, Hazle Township Municipal Building, 101 W. 27th Street, Hazle Township, Pa. until 12:00 Noon on Monday, November 8, 2021 for the following: 1. Emergency Snow Removal Services on an “on-call/as needed” basis during the 2022 Winter season. Single axle dump truck minimum 10 ton with plow, spreader & operator. Routes will be determined by the Road Master or Chairman of the Board. Insurance information and CDL information must be provided as per specifications. 2. Furnish and delivery at or near the Hazle Township vicinity by means of an unattended twenty-four (24) hour automated fueling facility utilizing vehicular cards and driver access cards, of 10,000 gallons of unleaded gasoline, more or less (minimum octane rating of.89). 3. Furnish and delivery at or near the Hazle Township vicinity by means of an unattended twenty-four (24) hour automated fueling facility utilizing vehicular cards and driver access cards, of 10,000 gallons of diesel fuel, more or less. 4. PA DOT Certified Concrete Mix per s.y. or ton in quantities as needed. 5. Tack material per gallon in quantities as needed 6. Crack & Joint Sealing Materials, as needed 7. 30 inch ADS pipe, single wall w/couplers, in 20 ft. sections in quantities as needed 8. 2″ and 3″concrete risers for 4 ft. x 4 ft. catch basins, in quantities as needed Above items are being bid for a contract period beginning January 1, 2022 and ending December 31, 2022. Bids received by the time specified above will be opened and read aloud at the regularly scheduled Hazle Township Supervisors meeting scheduled for Monday, November 8, 2021 at 6:00 P.M. at the Hazle Township Commons Building, 103 W. 27th Street, Hazle Township, Pa. 18202. Proposal forms and all pertinent documentation may be obtained at the office of the Supervisors, 101 W. 27th Street, Hazle Township, Pa. between the hours of 7:00 A.M. and 1:00 P.M., Monday thru Friday or by calling the Supervisors office at 570-455-2039. Proposals must be upon forms furnished by the municipality and must be accompanied by a certified check or bid bond where applicable.
